What a forklift certification card actually is?

- A national credential that follows the worker to every jobsite without new evaluation
- Valid if only an online quiz was completed
OSHA’s rule is equipment-specific and site-specific. Someone trained on a sit-down Class I or IV truck still needs additional training and evaluation before running a reach truck, order picker, or telehandler.
What OSHA requires on the record?
OSHA does not dictate card size, color, or laminate. It does require a written certification that includes four items:
- Operator’s name
- Date of training
- Date of evaluation
- Identity of the person who trained or evaluated the operator

Most U.S. employers also add:
- Employer name and worksite
- Forklift class or truck types authorized (Classes I–VII)
- Evaluator signature
- Re-evaluation due date (usually three years from the last evaluation)
Keep a file copy. The wallet card is for the floor. The file copy is what an inspector will ask to see.
Training must happen before you print anything
A free printable card is the last step, not the first. OSHA requires three parts:
- Formal instruction — classroom, video, or online material covering truck controls, capacity, stability, fueling or charging, visibility, and workplace hazards.
- Practical training — demonstration and hands-on practice under a qualified trainer.
- Workplace evaluation — the trainer watches the operator use the actual truck in the actual workplace and judges competence.

Refresher training is required when an operator drives unsafely, is in an accident or near-miss, fails an evaluation, switches to a different type of truck, or faces a workplace change that affects safe operation. Performance must be evaluated at least once every three years.
Federal child labor rules also matter: in most non-agricultural jobs, workers under 18 may not operate a forklift.

Check the fields before you print. Many third-party templates say “license.” OSHA does not issue a forklift license. Edit the heading to “Forklift Operator Certification” or “Powered Industrial Truck Training Record,” and make sure the four OSHA fields are on the card.

How to fill a free printable card the right way?
- Confirm formal training is finished and documented.
- Complete a hands-on evaluation on each truck type the person will use.
- Enter the operator’s legal name exactly as it appears in payroll records.
- Record separate training and evaluation dates if they differ.
- Print the trainer’s or evaluator’s name. A signature helps, even though OSHA names identity rather than a wet signature.
- List truck classes or model types. “All forklifts” is too vague.
- Set a re-evaluation date no later than three years out.
- File a copy with the training packet. Give the operator the laminated card.
Do not backdate a card. Do not issue a card after an online course alone. Do not buy a “national OSHA card” from a site that skips workplace evaluation.
